Authorisation and Consent for the Use, Collection and Disclosure of Personal Data

1. The personal data that you submit via, or upload or save on, this portal (including personal data submitted upon account creation) may be used by Ang Mo Kio - Thye Hua Kwan Hospital to assess your suitability for any suitable vacancies which are available. However, this does not impose any obligation on Ang Mo Kio - Thye Hua Kwan Hospital to (a) ensure that you are considered for any vacancies or (b) search for or recommend any vacancies to you.
2. Personal data submitted, or uploaded or saved on, this portal may be retained by us for such permitted period under the Personal Data Protection Act 2012 of Singapore.
3. By creating an account, you confirm that you consent to the collection, use and disclosure of your personal data for the purposes stated above, as well as for the purposes which are essential for administering the employment relationship between you and Ang Mo Kio - Thye Hua Kwan Hospital and its service providers if you are successfully recruited.
4. I further consent to the compilation and/or reproduction of my Personal Data or any other information about me discovered or received for the Purpose, into reports or documents, either in physical or electronic format, for Ang Mo Kio - Thye Hua Kwan Hospital 's use.
5. I acknowledge that my consent shall be valid until I withdraw and understand that I have the right to withdraw my consent at any time by giving reasonable written notice to Ang Mo Kio - Thye Hua Kwan Hospital. I also acknowledge and agree that if I withdraw my consent, Ang Mo Kio - Thye Hua Kwan Hospital will not be able to proceed with my employment application.

7. I consent to Ang Mo Kio - Thye Hua Kwan Hospital retaining my job application and all information contained therein including all supporting documentation, for a period of 24 months, for Ang Mo Kio - Thye Hua Kwan Hospital to consider any future positions which may be available even if I am not successful in the current job application. I acknowledge that I may at any time request Ang Mo Kio - Thye Hua Kwan Hospital not to retain such information and for these to be destroyed or deleted.
8. I agree not to hold liable, and to release Ang Mo Kio - Thye Hua Kwan Hospital and its service providers from any liability under the Personal Data Protection Act 2012 or any other relevant Singapore laws and regulations or under the general rules on confidentiality for the collection, use, disclosure and processing of my Personal Data for the Purpose.
9. I understand that Ang Mo Kio - Thye Hua Kwan Hospital may make copies of this Form and I consent to all such copies being accepted with the same authority as the original.
